Provisioning Form

When an order is placed for Maximo or TRIRIGA SaaS, a provisioning form must be completed as part of the order. The following information will be required on the form:

  1. Customer Primary and Secondary technical contact names, email and phone numbers

  2. Indication if the order is an add-on or change to an existing agreement, or a new order

  3. For TRIRIGA, if it is for a Flex deployment (Note: all Maximo EAM SaaS deployments are Flex)

  4. If customer is migrating from on-premise to the cloud (Y/N)

  5. If the order is for a Federal (FedRAMP) deployment (Y/N)

  6. Country (Geography) where the majority of the users are located
    (This will help IBM SRE determine the primary IBM Cloud Data Center location)

  7. When the environments need to be provisioned (approximate date)
    Note: Provisioning lead time is 5-10 business days from the date the order is fully approved & processed by the SRE Project Office. It is also dependent on the complexity and number of environment(s) on the order. See main provisioning page for details

  8. Expected date of production site go live

  9. Additional languages to be installed other than the base language of English (up to 5 are allowed)

  10. Total number of sites to be provisioned
    Note: new SaaS orders include two (2) sites by default (One PROD and one NON-PROD)

  11. URL Prefixes (Company Name + Env) for each environment
    For example, for a company called Acme, for Maximo SaaS, the environment URLs might be:

https://acme.maximo.com

Production

https://acme-test.maximo.com

Test

https://acme-dev.maximo.com

Development

Please note:

  • No special characters are allowed in prefixes except for a hyphen (-)

  • No extra sub-domains are allowed in prefixes (for example acme.test.maximo.com)

  • Second and top level domain names (maximo.com and tririga.com) are controlled and defined by IBM. These are required and are non-negotiable.

Important:
URL prefixes, once defined and submitted on the provisioning form, CANNOT be changed after the order is processed and provisioned. Be please sure you have fully vetted and approved the URL prefixes for all environments. Any URL prefixes that need to be changed post-provisioning will require complete re-provisioning of the environment(s) - at customer expense.

12. Welcome Kit recipient names and emails. Please include all users who should receive the welcome kit.
This could include the customer’s business partner, IBM services or system integrators
